Shannon Clifton, President & CEO
Shannon Clifton has over 14 years’ experience in health care population management with an emphasis in readmission mitigation, community-based healthcare, telehealth, remote patient monitoring and patient-provider satisfaction.
As President of Care Navigation Services, Shannon is responsible for driving the strategic vision, enhancing existing solutions focused on improved patient access and quality care throughout the healthcare continuum, and optimizing both internal and external partnerships to improve patient-to-provider experience for all populations served.
Prior to her current position, Shannon was the Director of Connected Care at CHRISTUS Health, an expansive integrated delivery network that includes more than 35 hospitals, in which she successfully developed, implemented and expanded several care management and telehealth programs.
Shannon’s achievements and responsibilities during her tenure at CHRISTUS, which include care transitions, community-based care programs, remote patient monitoring, tele-neurology and school-based telemedicine, were strongly aligned with Triple Aim – developing sustainable programs having improved health care outcomes and patient experience; better population health; and, lower per capita cost of care.
Shannon has co-authored numerous abstracts, articles and national presentations related to population health management, was co-interviewed by The Advisory Board, and was a presenter at the 2015 HIMSS National Conference. Under the leadership of the CMIO, Shannon, in collaboration with fellow Care Navigation Services leader Ellen Ford-Barton, led an Institutional Review Board study assessing if Remote Patient Monitoring enhanced clinical workflow efficiencies while improving patient outcomes, resulting in being named one of only four national semifinalists for Health Informatics 2014 Innovator Award.
She earned her Master of Science studies at The University of Texas Medical Branch in Galveston and a Bachelor of Science degree at The University of Texas at San Antonio.
