Dr. John W. Yaft, MD, Medical Advisor
As Care Navigation Services’ Medical Advisor, John W. Yaft, MD ensures all clinical policies, practices and solutions developed on behalf of our provider-partners reflect best medical practice. In collaboration with the Leadership Team, Dr. Yaft advises on care coordination, quality assurance and compliance methods, as well as promotes a learning culture within the Company to reflect current standards.
Dr. Yaft has been practicing medicine since 2013 and has also served in both academic and consulting capacities. His interests in quality improvement, hospital efficiency and population health management evolved during his residency in Dallas, having been fortunate to treat a variety of patient populations. This experience validated that patient management opportunities exist in workflows, staffing and patient dispositions and that predictive analytics play a key role in achieving favorable hospital-based clinical-quality outcomes.
Dr. Yaft also has a passion for working with patients with unfavorable social determinants of care and strives to promote the value hospitals can afford in prioritizing a holistic approach to healthcare. In his dual capacity as Medical Director for both Hospital Medicine and Utilization Management, Dr. Yaft is currently involved in many innovative pilot projects focused on enhancing healthcare delivery. Additionally, he continues to seek ways to measure patient-centered outcomes and engage both patients and families in health management decision-making to create a truly shared process.
Dr. Yaft is a graduate of Saint Louis University School of Medicine and trained in internal medicine at UT Southwestern Medical Center in Dallas. He is board certified in internal medicine.
